AN INVITATION TO MAKE YOUR PHYSICAL EXAM MORE PRODUCTIVE
The physicians and physician assistants at River Road Medical Group are committed to finding better ways to improve health and medical care for our patients. We are offering "HowsYourHealth," which can help improve your ability to maintain a healthy lifestyle. Here is how it works.
- Find an Internet connection. You may use any computer, which has Internet access, at home, at a friend's home, or at the library. The survey takes about 10 minutes.
- To access the web site, click here.
- Click on the SURVEY option and follow the steps to the "How's Your Health" survey.
- The next page will ask for an access code. At the bottom of the page, click the link in the gray box that says "Click HERE if you do not have a code but would like to do the survey."
- You and your family members can complete the survey by selecting the appropriate age and sex. All of your answers are confidential - no name identification is required. Based on your answers, you will be given special information personalized to your needs.
- Upon completion, you will also receive an "Action Form" and other forms which will summarize your results. We encourage to share this with your doctor or physician assistant when you come in for your complete physical examination.
